Seasonal and Age Distribution of Adenovirus Gastroenteritis
in Children

ÖZET
Amaç: ‹nfantil dönem baflta olmak üzere çocukluk ça¤›nda, viral gastroenteritler önemli enfeksiyon hastal›klar›d›r.
‹nfantil gastroenteritlerde, adenoviruslar rotaviruslardan sonra ikinci s›kl›kta görülen etkenlerdir. Bu çal›flmada,
çocukluk ça¤›nda adenoviruslara ba¤l› geliflen gastroenteritlerin yafllara ve mevsimlere göre da¤›l›m› incelenmifltir.
Gereç ve Yöntem: Çal›flmada immünkromatografik yönteme dayanan, Adenovirus Rapid Test Device kitleri
(Meikang Biotech, Shanghai) ile gaita örneklerinde adenovirus aranm›flt›r.
Bulgular: Konya E¤itim ve Araflt›rma Hastanesi’ne 2009 y›l›nda baflvuran akut gastroenteritli 1329 çocuktan
al›nan gaita örneklerinin % 4.4’ünde (58 örnek) adenovirus serotip 40 ve 41 antijeni saptanm›flt›r. Adenovirus
saptanan çocuklar›n yafllar› 5 ay-16 yafl aras›nda olup, en yüksek pozitiflik oran› 47 vaka ile (%81) 0-2 yafl
aras›nda bulunmufltur. ‹shalin mevsimlerden sonbahar (% 5.4), aylardan ise May›s (% 6.7) ay›nda en s›k oldu¤u
saptanm›fl ancak mevsimler ve aylar aras›nda s›kl›k aç›s›ndan istatistiksel olarak anlaml› bir fark görülmemifltir.
Sonuç: Akut gastroenteritlerde adenovirüslar›n saptanmas›, a¤›r seyirli klini¤e sahip süt çocuklar›nda gereksiz
yere antibiyotik kullan›lmas›n›n engellenmesi aç›s›ndan önemlidir.

SUMMARY
Objective: Viral gastroenteritis is among the significant infectious diseases in childhood period, especially in
infants. Among the causative agents of infantile viral gastroenteritis, adenoviruses rank second to rotaviruses. In
this study the seasonal and age distribution of adenovirus gastroenteritis in children were investigated.
Material and Methods: Adenovirus was investigated in the stool samples of children with acute gastroenteritis
by using Adenovirus Rapid Test Device (Meikang Biotech, Shanghai) which was based on immunochromatography.
Results: Among 1329 children admitted to Konya Research and Education Hospital in 2009 with acute gastroenteritis,
58 (4.4 %) revealed adenovirus serotype 40 and 41 antigen in their stool samples. The age range of children with
adenovirus was between 5 months to 16 years. Most of the cases were between 0 and 2 years (n=47; 81 %).
Although diarrhea was most frequent during autumn (5.4 %) and also in May (6.7 %), there was no significant
statistical difference in incidence of adenovirus gastroenteritis in terms of seasons and months.
Conclusion: The detection of adenoviruses in severe acute gastroenteritis in infants is important in order to
prevent unnecessary use of antibiotics.

Enfeksiyöz diareler tüm dünyada, solunum yolu
enfeksiyonlar›ndan sonra ikinci s›kl›kta hastal›k
ve ölüm nedeni olarak karfl›m›za ç›kmaktad›r
(1). Akut gastroenteritler, tüm dünyada küçük
çocuklardaki mortalitenin major nedeni olup bu
ça¤daki ishallerin büyük bir k›sm›ndan virüsler
sorumludur (2,3). Geliflmekte olan ülkelerde
gastroenteritlerde bakteriyel ajanlar ön planda
oldu¤undan viral etkenlere dayal› oluflan
gastro-enteritlerin önemi fazla irdelenmemektedir.
Vi-ral etkenler ise hem geliflmifl, hem de
geliflmek-te olan ülkelerde, özellikle infant ve erken
ço-cukluk dönemindeki gastroenteritlerin önemli
etkenleri aras›ndad›r (1,4). Çocukluk ça¤›
gast-roenteritlerinin viral etiyolojisinde
adenovirus-lar, rotaviruslardan sonra ikinci s›kl›kta
görül-mektedirler (5,6).
Günümüzde adenoviruslar›n 41 farkl› serotipi
tan›mlanm›flt›r (5). En s›k gastroenterit yapan
serotipleri, serotip 40 ve 41’dir (5-8). Enterik
adenoviruslara ba¤l› enfeksiyonlar›n büyük
ço-¤unlu¤u 2 yafl alt›nda görülmektedir (9-11).
Özellikle 6 ay›n alt›ndaki bebeklerde daha
s›k-t›r. Hatta Richmond ve arkadafllar› (12) bu yafl
grubunda adenovirus gastroenteritlerinin
rotavi-ruslardan daha s›k görüldü¤ünü bildirmifllerdir.
Ancak birçok farkl› araflt›rmac› alt› ay›n
alt›nda-ki çocuklarda adenovirus gastroenteritlerinin,
rotaviruslardan sonra ikinci s›kl›kta görüldü¤ü
görüflünü savunmufllard›r (6,9,13,14).
Ülkemiz-de ise bu konudaki araflt›rmalar çok k›s›tl›
dü-zeydedir. Çünkü bu virüsler, konvansiyonel
hücre kültürlerinde üretilemedi¤inden tan›da
büyük güçlüklerle karfl›lafl›lmaktad›r.
‹mmun elektron mikroskobunun keflfiyle son
y›llarda, viral partiküllerin morfolojik ve
immü-nolojik ay›r›mlar› yap›labilmifl ve çocukluk ça¤›
gastroenteritlerinin viral etyolojisi hakk›nda
da-ha çok bilgiye ulafl›labilmifltir (5,7,15,16).
Ade-novirus antijenleri, grup reaktif poliklonal ya da
monoklonal antikorlar kullan›larak gösterilebilir
(17-20). Gaita örne¤inde hem grup spesifik
(adenovirus tip 3 hexon antijeni), hem de tip
spesifik (tip 40 ve 41) antijenler çeflitli yöntemler
ile tespit edilebilmektedir. Bu amaçla, ELISA,
lateks aglütinasyon, immunkromatografi ve
moleküler yöntemler kullan›lmaktad›r (21,22).
GEREÇ VE YÖNTEM
Hastanemize 2009 y›l› içinde akut gastroenterit
flikayetiyle baflvuran 0-16 yafl aras›ndaki
çocuk-lar›n 1329’undan al›nan taze d›flk› örne¤inde
adenovirus antijeni araflt›r›lm›flt›r. Çal›flmada,
immünkromatografik yöntemle çal›flan, adenovirus
40 ve 41 serotip antijenini tan›yan Adenovirus
Rapid Test Device (Meikang Biotech, Shanghai)
kitleri kullan›lm›flt›r.
Kat› gaita örne¤inden yaklafl›k 50 mg, s›v› gaita
örne¤inden ise iki damla (yaklafl›k 50 µL)
al›na-rak ekstal›na-raksiyon tamponu içinde kar›flt›r›lm›fl ve
haz›rlanan kar›fl›m bir saat içinde çal›fl›lm›flt›r.
Bu kar›fl›ma stripler dald›r›larak test 10 dakika
içinde de¤erlendirilmifl; 20 dakikadan sonraki
sonuçlar de¤erlendirmeye al›nmam›flt›r.
Striplerin üzerinde kontrol ve test band›n›n
bir-likte oluflmas› pozitif, sadece kontrol band›n›n
oluflmas› negatif sonuç olarak de¤erlendirilmifl;
kontrol band› oluflmay›p sadece test band›n›n
oluflmas› durumunda ise test de¤erlendirilmeye
al›nmam›flt›r.
Uygulama ve de¤erlendirmede kiflisel farkl›l›k
olmamas› için testler ayn› teknisyen taraf›ndan
uygulanm›fl ve sonuçlar hastane otomasyonuna
kaydedilmifltir.
Yap›lan istatistiksel de¤erlendirmede SPSS 15.0
paket program› kullan›lm›fl ve veriler ki-kare ve
Mc Nemar analizleri ile yorumlanm›flt›r.
BULGULAR
Hastanemize 2009 y›l›nda baflvuran akut
gastro-enteritli 1329 çocuktan al›nan gaita
örneklerin-de, 58 (% 4.4) çocukta adenovirus serotip 40 ve
41 antijeni saptanm›flt›r.
Adenovirus saptanan 37 erkek, 21 k›z
çocu¤u-nun yafllar› 5 ay-16 yafl aras›nda olup yafl
ortala-malar› 2.12±1.81 olarak bulunmufl, en yüksek
pozitiflik oran› 47 vaka ile (%81) 0-2 yafl
ara-s›nda görülmüfltür (Tablo 1).
‹shalin en s›k görüldü¤ü mevsim sonbahar iken
(%5.4), en s›k görüldü¤ü ay ise may›s ay›
(%6.7) olarak saptanm›fl (Tablo 2), ancak
ade-novirus görülme s›kl›¤› ile mevsimler ve aylar
aras›nda istatistiksel olarak anlaml› bir fark
bu-lunamam›flt›r (p>0.05).
TARTIfiMA
Viral gastroenteritler, özellikle infant ve erken
çocukluk döneminin önemli hastal›lar›
aras›nda-d›r (1,4). Çocukluk ça¤› gastroenteritlerinin
vi-ral etiyolojisinde adenoviruslar, rotaviruslardan
sonra ikinci s›kl›kta görülmektedirler (5,6).
Farkl› ülkelerde yap›lan çal›flmalarda
adenovi-rus gastroenteritlerin s›kl›¤› % 2.4-22.2 oranlar›
aras›nda (9,11,23-29), ülkemizde ise % 4.5-16.2
oranlar› aras›nda bildirilmifltir (30-39).
Avrupa, Asya, Kuzey ve Güney Amerika’da
yap›-lan çal›flmalar sonucunda enterik adenovirüslerin
çocukluk ça¤› gastroenteritlerinin % 3.1 ile %
13.5’inde etken oldu¤u gösterilmifltir(40-43).
Ül-kemizde 2 yafl alt› çocuk ishallerinde % 10
ora-n›nda adenovirus saptanm›flt›r (38,44). Rotavirus
enfeksiyonlar› daha çok k›fl ve ilkbahar aylar›nda
görülmekte (9,45-47) ancak enterik
adenovirusla-r›n mevsimsel da¤›l›m› de¤ifliklik göstermekte ve
tüm y›l boyunca görülebilmektedir (14,47).
Ade-novirüsler üst solunum yolu enfeksiyonu
s›ras›n-da ve sonras›ns›ras›n-da d›flk›s›ras›n-da bulunabilirler ancak
sa-dece serotip 40 ve 41, daha nadir olarak da
sero-tip 31, gastroenterite neden olur (14,48).

Küçük çocuklara gruplar halinde bak›lan
yuva-lar (baflta alt› ay-iki yafl aras›ndaki çocukyuva-lar
ol-mak üzere) gastroenterit bak›m›ndan daha
yük-sek risk tafl›maktad›r. Adenovirusun kuluçka
sü-resi 3-10 gündür (48). Adenovirüs enteriti
s›k-l›kla 10-14 gün gibi uzun süren ishale neden
olur (14). Adenovirus gastroenteritinin di¤er
is-hallere göre anlaml› olarak daha uzun sürdü¤ü
bildirilmifltir (26,49). Çal›flmam›zda adenovirus
gastroenteritleri en s›k 0-2 yafl grubunda (% 81)
saptanm›fl ve uyumlu olarak birçok araflt›rmac›
da adenovirus gastroenteritlerini en s›k 0-2 yafl
grubunda % 54.9-85.7 oranlar› aras›nda
bul-mufllard›r (31,34,35,50).
Adenovirus gastroenteritleri tüm y›l boyunca
görülebildi¤i gibi baz› araflt›rmac›lar en s›k k›fl
ve yaz aylar›nda oldu¤unu bildirmifllerdir
(30,31,34,36). Çal›flmam›zda ise en s›k
sonba-har ve ilkbasonba-har mevsiminde görülmüfltür. En s›k
görülen aylar ise s›ras›yla May›s (% 6.7),
Ka-s›m (% 5.8), Haziran (% 5.6), Ekim (% 5.6) ve
A¤ustos (% 5.1) olarak bulunmufltur.
Çal›flma-m›z›n istatistiksel de¤erlendirmesinde
yayg›nl›-¤›n aylar ve mevsimlerle iliflkili olmad›¤›
sap-tanm›fl ve çal›flmam›zla uyumlu olarak bir çok
araflt›rmac›n›n çal›flmas›nda da yayg›nl›¤›n
ay-lara göre de¤iflti¤i ve y›l›n her mevsiminde
farkl› oranlarda saptand›¤› bildirilmifltir
(11,30,31,35,39).
Yap›lan birçok çal›flmada yaz aylar›nda biraz
daha s›k görüldü¤ü bildirilmifltir (9,24,48).
An-cak Mickan ve arkadafllar› (51) 1994 y›l›nda
yapt›klar› bir çal›flmada adenovirus tip 40'›
ilk-bahar ve k›fl, tip 41'i ise daha çok yaz
dönemle-rinde saptad›klar›n›, non-enterik tipleri ise k›fl
sonu ve ilkbaharda daha s›k tespit ettiklerini
bil-dirmifllerdir. Bizim çal›flmam›z bir y›ll›k
döne-mi kapsamaktad›r. Daha güvenilir sonuçlar için
daha uzun süreli çal›flmalara ihtiyaç vard›r.
Ni-tekim Bates ve arkadafllar› (9) befl y›l süreli
ça-l›flmalar›nda enterik adenoviruslar›n mevsimsel
de¤ifliklik göstermedi¤ini tespit etmifllerdir.
Sonuç olarak, viral gastroenteritlerin bakteri
ve-ya protozoonlara ba¤l› gastroenteritlerden klinik
olarak ay›r›m› güçtür. Bu aflamada klinik teflhisi
do¤rulamak için laboratuvar deste¤i
gerekmek-tedir (34,52). Yapt›¤›m›z çal›flmada birçok
yer-de kullan›lan immunkromatografik yöntem ile
antijen tespiti yap›lm›flt›r. Antijen tespiti için
la-teks aglütinasyonu da yayg›n kullan›lmakla
be-raber, son y›llarda immunkromatografik yöntem
giderek yayg›nlaflm›flt›r (31,34,39,53). Bu
yön-temin, ELISA ile uyumlu olmas›, 5-10 dakika
gibi k›sa sürede sonuçlanmas›, az miktardaki
d›flk› örne¤iyle kolayl›kla çal›fl›labilmesi ve
du-yarl›l›¤›n›n yüksek (%93-100) olmas› gibi
avan-tajlar› vard›r. Ancak, yeni do¤anlarda ve altta
yatan intestinal hastal›¤› olanlarda yanl›fl
pozi-tiflik verebilece¤i de unutulmamal›d›r (14,48).
Akut gastroenteritlerde viral etkenlerin (ör;
ro-tavirüs, adenovirüs) belirlenebilmesi, özellikle
süt çocuklar›nda a¤›r seyirli klini¤e sahip
hasta-larda tedavi yaklafl›m› aç›s›ndan önemlidir.
Böylece gereksiz yere antibiyotik kullan›lmas›
da engellenmifl olacakt›r.
