• Çalışmamız kronik sinüzitli hastalarda fonksiyonel endoskopik sinüs cerrahisi sonrası sinonazal kavitede kullanılan trombositten zengin fibrinin etkileri klinik olarak araştırılmıştır ve literatürde benzer klinik çalışma mevcut değildir. Çalışmamız sonuçları ile literatüre katkı sağlamaktadır.

• Trombositten zengin fibrinin kullanımının operasyon sonrası erken dönemde hastaların subjektif semptomlarında iyileşmesi üzerinde etkili olmadığı görülmüştür. Her iki hasta grubunda da hasta semptomlarının uygulanan fonksiyonel endoskopik sinüs cerrahisi ile başarılı olarak giderilmesinin sübjektif yanıtları etkilediği düşünülmüştür.

• Cerrahi sırasında sinonazal kavitede kullanılan trombositten zengin fibrinin hemostaz ile mikrovasküler kanamaları azalttığı izlenmiştir.

• Operasyon sonrası birinci haftada yapılan endoskopik nazal muayene evrelemesinde trombositten zengin fibrin uygulanan hasta grubunda belirgin olarak ödem ve akıntı/kurut miktarında azlık izlenmiştir. Bu hasta grubunda sağlıklı epitelizasyonun başladığı görülmüştür.

• Operasyon sonrası birinci ayda yapılan endoskopik nazal muayene evrelemesinde iki hasta gurubu arasında ödem ve akıntı/kurut miktarında farklılık saptanmamıştır. • Eriyebilen selüloz tamponların kullanıldığı kontrol hasta grubu ile

karşılaştırıldığında erken dönemde ödem ve kurum miktarında azlık saptanmıştır. • Cerrahi sonrasında mikrovasküler kanamaların kontrolünde eriyebilen selüloz

tamponlara alternatif olabileceği sonucuna varılmıştır.

• Operasyon sonrası erken dönemde cerrahi debridmanın düzenli ve etkili olarak uygulanamayacağı hasta grubunda TZF emilebilen selüloz bazlı tamponlara alternatif olarak kullanılabilir.

• Çalışma ve kontrol gruplarında koku kaybında operasyon sonrası belirgin iyileşme saptanmış fakat iki grup arasında farklılık saptanmamıştır. Trombositten zengin kan ürünlerinin koku üzerine etkilerinin araştırılması için objektif testlerin kullanıldığı uzun takip süreli klinik çalışmalara ihtiyaç duyulmaktadır.

• Hazırlanması maliyet gerektirmeyen bir otolog kan ürünü olan trombositten zengin fibrinin kronik sinüzite yönelik cerrahiler sonrasında erken dönem epitel iyileşmesinin indüklenmesi ve mikrovasküler kanama kontrolü için kullanımı önerilmektedir.
