AUTHORSHIP FORM

Authorship criteria is based on the International Committee of Medical Journal Editors (ICMJE) Uniform Requirements for Manuscripts Submitted to Biomedical Journals.

The ICMJE lists four conditions for authorship credit. Authors must meet all four conditions in order to be listed.

• Substantial contributions to conception and design, acquisition of data, or analysis and interpretation of data, and

• Drafting the article or revising it critically for important intellectual content, and

• Final approval of the version to be published, and

• Agreement to be accountable for all aspects of the work in ensuring that questions related to the accuracy or integrity of any part of the work are appropriately investigated and resolved.

The order of authorship in the byline should be a joint decision of the co-authors. Each author should participate sufficiently in the work to take public responsibility for his or her contributions to the study, the content and for any conflict of interests.

Authorship is not justified for individuals who simply obtain or provide funding, participate in data collection or general supervision of the research, or serve as head of the group, though such contributors may be acknowledged with their written permission in the Acknowledgements section.
