肥胖婦女減重後對體組成及血脂質之影響.

Abstract

體重過重或肥胖常與許多慢性疾病的發生或嚴重程度有關,而減重可使其疾病症狀減輕或消除。 以往我國關於肥胖之研究多為流行病學方面之研究,而對於減重前後變化之比較則探討較少,因 此本實驗之目的在研究 67 個停經前之肥胖女性,依其肥胖程度將身體質量指數(BMI)值 264-30.8 者分為輕度肥胖組,而BMI>30.8 以上者分為中度肥胖組,以飲食控制方式減輕體重,追蹤半年, 比較減重前後體組成及血脂質之變化情形。在 67 個受測者中選取體重減輕大於原體重 5%以上 者 29 人,並選取與減重者年齡身高相配合之正常體重者 18 人做為控制組,加以分析比較。結果 顯示:不同程度肥胖女性,不管是體重、瘦體組織及脂肪組織重量皆有顯著之不同,且中度肥胖 組大於輕度肥胖組,輕度肥胖組又大於正常體重組。肥胖者減重大於 5%之 29 人中,減重後體 重平均下降 9.2±5.2kg(由 79.6±13.8 kg下降至 70.4±11.2kg),脂肪組織含量百分比下降 3.6±2.3% (由 35.8±3.6%下降至 32.2±3.4%),均有顯著差異(P<0.05)。在血脂質上,中度肥胖組血漿三酸 甘油酯(TG)、總膽固醇(total cholesterol, TC)、低密度脂蛋白膽固醇(LDL-C)、LDL-C與高密度脂 蛋白膽固醇(HDL-C)之比值皆顯著較輕度肥胖者為高,而HDL-C則顯著較低。血漿TG、TC、 LDL-C、及LDL-C/HDL-C在減重後均顯著較減重前降低,HDL-C則顯著上升(P<0.05),但HDL-C 濃度肥胖組仍顯著較正常體重組為低。本研究結果顯示體重減輕只要大於原體重 5%以上,即可 有效改善血脂質組成。不同程度減重組別間血脂質則沒有顯著差異。
