T h e L a n c e t 1 9 9 8 ;3 5 2 :8 7 -9 3 .Coronary stenting with the use of heparin, aspirin and ticlopidin for thrombo-prophylaxis is performed in more than 5 0 0 .0 0 0 patients per year. In this randomised, controlled trial, 2 3 9 9 patients with ischemic heart disease and suitable coronary artery lesions w ere assigned to stenting plus placebo (n:809), stenting plus abcixlm ab, a ll-b /lll-a inhibitor, (n:794) or balloon an g io p lasty plus ab cixim ab (n :7 9 6 ). All patients re ce iv ed hep arin , aspirin and standard pharm aco log ical th erapy. T h e prim ary end point (d eath , m yocardial infarction or need for urgent revascularization in the first month) occurred in 87 (1 0 .8 % ) of 8 0 9 patients in the stent plus placebo group, 4 2 (5 .3 % ) of 7 9 4 in the stent plus abciximab group and 5 5 (6 .9 % ) of 7 9 6 in the balloon plus abcixim ab group. T he main outcomes that ocurred less with abcixim ab w ere death and large myocardial
infarction. In conclusion, platelet glycoprotein -ll-b /lll-a blockade with abciximab substantially improves the safety of coronary stenting procedures and balloon angioplasty with abcixim ab is safer than stenting without abciximab.

In patients with heart failure, B-blokade has improved morbidity and left ventricular function, but the impact on survival is uncertain. In this multicentre, double blind, random ised, p lacebo-controlled trial, 2 6 4 7 symptomatic patients in N Y H A class III or IV, with left ventricular ejection fraction of 35 % or less, receiving diuretics and A C E -in h ib itors w ere investigated. Patients w ere randomly given either bisoprolol 1.25 m g(n:1327) or placebo (n:1320) daily, the drug being progressively increased to a maximum of 10 mg per day. The study was stopped early, because bisoprolol showed a significant benefit com pared to placebo. All cause mortality was significantly lower with bisoprolol than on placebo (156 (11 .8 % ) vs 2 2 8 (1 7 .3 % )). There w ere also significantly few er sudden deaths among patients on bisoprolol than in those on placebo. In conclusion, B-blokade with bisoprolol significantly improves survival in patients with severe congestive heart failure.

The neuronal ceroid lipofuscinoses are a group of progressive en cephalopathies which are inherited in an autosomal recessive m anner. Four main types are recognized: infantile, late infantile, juvenile and adult.
Morphologically they are characterized by w idespread accumulation of autofluorescent lipopigments inside and outside the central nervous system. In the infantile type the lipopigments are granular. T he late infantile type shows characteristic curvilinear profiles. Fingerprint profiles are seen in the classical juvenile type while the adult type can be som ewhat variable showing granular material and/or fingerprint profiles. M any cell types contain the abnormal inclusions including endothelial cells and amniotic fluid cells. In som e subtypes this has allow ed for prenatal diagnosis by ultrastructural exam ination of uncultured amniotic cells (late infantile type) and endothelial cells of biopsied chorionic stromal vessels (infantile type) (1).
