NOTLAR/NOTES
A. Genel Notlar/General Notes 1 Form okunaklı olarak tercihen daktilo ile doldurulacaktır. Formda silinti veya üst üste kelimeler bulunmamalıdır.
Düzeltmeler, yanlış kelimenin üstü çizilerek doğrusunun yazılması suretiyle
yapılır. Düzeltmeler belgeyi dolduran kişi tarafından imzalanıp, 16 nolu kutuyu dolduran gümrük idaresince mühürlenmek suretiyle onaylanır. The form
must be completed legibly and inedibly, preferably by typewriter. It must not
contain any erasures or ovenwritten words. Corrections should be made by crossing out wrong entries and if necessary adding the correct particulars.
Corrections must be intialled by the person completing the form and endorsed
by the customs office which copletes box 16. 2 1 ila 15 nolu kutular başvuru
sahibi tarafından doldurulur. Boxes 1 to 15 must be completed by the holder.
B. Bazı Kutulara İlişkin Özel Notlar/Special notes referring to box numbers 1
İlgili kişinin adı, adresi ve ülkeyi yazınız. Tüzel kişilik olması halinde, yetkili kişinin ad ve soyadı yazılmalıdır. Give the name, address and the name
of country. In the case of a legal person, the name of the person responsible
should also be given. 3 İsim, açık adres ve ülke adını yazınız. Give the name,
address and the country. 6 İznin sayı ve tarihi ile izni veren idareyi yazınız. Give the number and date of the authorisation and the name of the customs authorities which issued it. 10 Normal ticari adı veya tarife tanımını
kullanarak işlem görmüş ürünleri yazınız. Give an exect decription of the compensating products using the normal commercial description or the tariff description. 11 İzin belgesinde gösterilen şekilde, işlem görmüş ürünlerin tarifesini yazınız. Give the tariff heading or subheading of the
compensating
products as shown on the authorisation. 12 Normal ticari adı veya tarife tanımını kullanarak eşyayı yazınız. Tanım ihracat belgesinde yer alan tanımla
benzer olmalıdır. Eğer eşya dahilde işleme işlemine tabi ise, "D.İ.R.
eşyası"nı yazınız ve eğer kullanıldıysa INF 1 belgesinin numarasını yazınız.
Give an exact description of the goods using the normal commercial description
or the tariff description. The description must correspond with that given in
the export document. If the goods are subject to the inward processing procedure enter 'IP goods' and give the number of the information sheet INF 1
if used. 14 Metrik sistem birimleri (kg. lt, m2, vs.) ile ifade edilmiş net miktarı yazınız. Give the net quantity expressed in units of the metric system
(kg, litres, mg, etc.) 15 İhracat beyannamesi sunulurken ki istatistiki kıymeti aşağıdaki döviz kısaltmaları önde olmak üzere yazınız. Give the statistical value at the time the export declaration was lodged, preceded by

1. Düzenlenen INF 4 Bilgi Formu, mekanik kağıt hamuru içermeyen, yazım boyutunda olan ve ağırlığı 40 ila 65gr/m2'den az olmayan beyaz kağıt
üzerine basılır.
2. Her bir form 210x297 mm. boyutlarında olmalıdır
3. Müsteşarlık formları bizzat basmak veya bastırmak hakkını saklı tutur.
Form ayrıca kendisini tanıtıcı mahiyette bir seri numarası taşımalıdır.
