2.3 Atık Ürünlerin Yapı Malzemesi Olarak Değerlendirilmesi

A cárie dentária pode ser considerada uma lesão bucal comum em pacientes com HIV, uma vez que apresentam higiene bucal deficiente e fluxo salivar reduzido. Fatores estes que colaboram para o aumento na prevalência de cárie (DIZ DIOS et al., 1999). No presente estudo os adolescentes com HIV apresentaram maior CPOD e maior risco de apresentar cárie dentária quando comparado com os adolescentes do grupo controle.

Estudo realizado no Zimbabué, avaliando 156 pacientes com AIDS de várias faixas etárias, encontrou maior prevalência de cáries de progressão rápida em crianças (CHIDZONGA, 2003). Em Bogotá, foram avaliadas as condições bucais em 37 adolescentes nascidos com HIV e os resultados mostraram que 45,9% apresentavam cáries ativas e a condição bucal apresentou-se insatisfatória devido a grande necessidade de tratamento odontológico (DAVILA & GIL, 2011). Os resultados do presente estudo demonstraram que os adolescentes utilizando a medicação terapia antirretroviral altamente ativa apresentaram uma pior condição de cárie dentária em relação aos adolescentes sem a doença AIDS, situação semelhante occoreu em estudo desenvolvido em Moçambique, que mostrou maior prevalência da cárie nos adolescentes que estavam utilizando a terapia antirretroviral altamente ativa SALES-PERES et al,.2012).

Ao analisar o índice CPOD, em vários estudos no continente Africano, identificou-se que o componente cariado aparece como o mais prevalente entre os componentes CPOD, seguido do perdido e por último o restaurado (MAPENGO et al., 2010; BAJOMO et al., 2004; VAN WYK & VAN WYK, 2004). O presente estudo apresentou a mesma ordem, o que mostra o baixo acesso dessa população aos serviços odontológicos em Maputo, componente restaurado nos pacientes com AIDS foi encontrada o que ilustra uma descriminação ou falta de atenção por partes dos dentistas neste tipo de pacientes.

Em relação aos livres de cárie, 22,67% e 32,35% dos adolescentes com HIV e do grupo controle, respectivamente, não apresentaram história de cárie dentária, Valores semelhantes foram encontrados por Mapengo et al (2010) em adolescentes que não eram portadores de HIV/AIDS e por Birnbaum et al (2002) em portadores da doença. Pesquisa realizada em 83 adolescentes com AIDS, no Zaire, constatou que 68,4% dos adolescentes livres de cárie dentária (TUKUTUKU et al., 1990).

62 Discussão

É comum a prescrição de dieta hipercalórica para crianças infectadas por HIV e com atraso no crescimento (GELBIER et al 2000). Essa alta ingestão de carboidratos por essas crianças pode estar associada ao aumento de microrganismos cariogênicos e consequentemente maior prevalência da cárie (MADIGAN, 1996). Entretanto, Madigan et al (1996) não observaram maior ingestão de carboidratos em crianças infectadas quando comparado com os irmãos sadios, sugerindo assim que as primeiras estariam mais susceptíveis ao efeito de ingestão de açúcares devido ao reduzido fluxo salivar ou a outros fatores predisponentes. Estudos futuros devem ser conduzidos para esclarecer melhor essa relação.

A redução na prevalência da cárie dentária não ocorre de forma homogênea, se concentra em pequenos grupos, sendo denominada polarização da cárie dentária. O termo “polarização de cárie” foi definido como as extremidades da doenças, de um lado a ausência e do outro doença concentrada em um pequeno número de adolescentes (NARVAI et al., 2006). Este fenômeno pode ser avaliado por meio do índice SIC (Significant Index Caries).

Este estudo reitera esses achados, uma vez que o SIC encontrado nos adolescentes com AIDS foi de 6,17±3,05 e nos adolescentes sem a doença foi de 6,08±2,76. Valor similar ao encontrado no estudo realizado por Mapengo et al (2010), em Maputo Moçambique nas áreas urbanas e suburbanas em adolescentes de 12 anos. Dessa forma, este estudo ressalta a necessidade de melhorar os programas preventivos e educativos oferecidos aos escolares e pré-escolares, sinalizando a necessidade de tratamento e fluoretação da água de abastecimento público. Dentro do programa de atenção à saúde do paciente portador de HIV/AIDS devem ser incluídas consultas odontológicas junto às consultas médicas de acompanhamento, sendo então inserida a presença do cirurgião dentista na equipe multidisciplinar, melhorando a qualidade de vida desses adolescentes.

7 CONCLUSÕES

O IMC normal foi mais prevalente em adolescentes com AIDS. Já no grupo controle, a condição de baixo peso foi mais prevalente.

A doença periodontal, alteração do tecido mole e IMC não foram encontradas diferenças estatisticamente significativas entre os grupos com e sem AIDS.

A cárie dentária apresentou com maior intensidade em indivíduos com AIDS. As condições bucais e IMC dos adolescentes com AIDS não apresentou diferenças estatisticamente significativas com o nível de contagem de linfócitos TCD4+.
