PleurX ascites kateter

Ju tidigare i sjukdomsförloppet katetern läggs, desto större nytta för patienten. I en stor studie har man valt att lägga PleurX vid första terapeutiska tappningstillfället på patienter som har nydiagnosticerad malign ascites med snabb ackumulation,

besvärande symptom och dålig livskvalitet då upprepade paracenteser skulle vara obekvämt för patienten. Citat ur studien:

”Furthermore, repetition of this procedure (LVP, reds anm) leads to frequent hospital visits, which are inconvenient for the patient, entail the use of more healthcare

resources, and may lead to delays in drainage resulting in patients becoming

progressively symptomatic in the terminal stages of their illnesses. Given that over 80 percent of the patients in this study had poor performance status (an ECOG score of at least 3), in our clinic we used clinical judgement that the PleurX catheter would benefit the patient – rather than a history of therapeutic paracentesis – as another indication for catheter insertion. This was justified by the low rate of catheter removal (4,05%), as mentioned earlier”.

PleurX förslag på dränageprotokoll vid ascites

1. Implantera PleurX och tappa så mycket som är möjligt vid inläggningstillfället.

Observera nationella riktlinjer vid ascitestappning vad gäller eventuell albuminersättning.

2. Patienten kan gå hem samma dag och tappas på max 2000 ml per dygn.

3. För att undvika läckage rekommenderas tätare tappningar de första veckorna tills manschetten på katetern har vuxit fast.

4. Dränage 2-3 gånger i veckan samt vid behov med ewimed dränageset 2000 ml.

TIPS

• Tänk på att dränagepåsen ska kopplas loss efter varje tappning och ett nytt ventillock ska placeras på kateterns säkerhetsventil.

• Patient alternativt hemsjukvårdspersonal måste vara utbildad i hur man dränerar med aseptisk teknik. Kontakta gärna fenik för utbildning.

• Skriv en tydlig ordination för suturtagning, så att fixeringsstygnen inte dras av misstag innan 30 dagar har gått.

• Förbandet ska bytas 2 gånger per vecka.
